2012 REGISTRATION PROCESS

All camp applications are now available for 2012!  You can register online, or download & mail in the forms found below.

Please note: Online registration requires a small extra fee (5.5%) that will be attached to your total cost via credit card or check.

DISCOUNTS

Sibling discount: 10%

Team discount: 10%

  •           Must be 6 or more players from the same team
  •           Mail-in registration is preferred but not required
  •           Please write on application the TEAM NAME and TEAMMATE NAMES to receive discount

    To reserve your spot at camp, please fill out an application form and return to the address below with a $175 deposit (only $100 for winter camp):

    CONFIRMATION

    Upon receipt of your application form and camp deposit, applicant will receive confirmation of enrollment in camp via email. Included in the confirmation email will be an update of any outstanding forms and/or remaining balance needed to complete the registration process.


    REGISTRATION CHECK-LIST

    Completed registration includes the following, preferably returned two (2) weeks prior to the start of camp:

    • Application
    • Deposit ($100/$175)
    • Health Forms (4 Pages)
      1.  Parent/Guardian Consent and Release form
      2.  Health Insurance Information Sheet
      3.  Summer Camp Health Questionnaire
      4.  Physical Exam Information


    CHECK-IN

    Indoor Camp: Registration and camp will be held at Oosterbaan Fieldhouse.  Please check-in at the registration table and turn in any remaining balance and health forms.

    Summer Camps; Registration and camps will be held at the U-M Soccer Complex.  Please check-in at the registration table and turn in any remaining balance and health forms.
